Your initial appointment may consist of a very comprehensive oral examination that includes hard and soft tissue analysis, temporo-mandibular joint (TMJ) exam, occlusal analysis and a complimentary esthetic assessment.
What we will need when you come for your first visit:- A release form signed by you to allow us to transfer any x-rays or medical history files from your previous dental office.
- A completed New Patient Information form
